Advancing Plan for Health Service Development in SichuanProvince
Health service is the most fundamental, direct,realistic and irreplaceable rigid demand for individuals. This system consiststhe promotion in many areas, including the general and special medical andhealth service and facilities for public health, disease diagnosis, and diseasetreatment; the taking-up medical service and facilities for chronic diseasemanagement, recovery after operations, long-term nursing for the disabled and Alzheimerpatients, and palliative care; the health management and promotion service forhealth management as well as advisory, physical check, traditional Chinesemedicine health cultivation. To attain the goal of protecting and improvingpeople’s physical and mental health, the health service industry mainlyinvolves the relevant services including medical service, health management andimprovement, health insurance, etc. It is an industry group involving multiplesectors, such as pharmaceuticals, medical devices, health-care products, health-carefood, physical training equipment, and physical exercise. The evolution in thisindustry is the government’s public responsibilities to improve people’slivelihood, ensure people’s rights to “medical care and elderly support”,promote social fairness and justice. Additionally, it represents a startingpoint of breakthrough to foster new economic growth point, expand expenditure,and accelerate economic transitioning as well as upgrading. Such sector is alsoa sunrise industry in our Province blessed with great prospects. This actionplan is specifically formulated to encourage the growth of such sector.
1.Significance
1) Encourage theeconomic growth. Health service plays an essential role in modern serviceindustry. In many developed countries and regions, the industry has enteredinto the stage of maturity and has made huge contribution for national economy.Currently, health service has become one of the largest industries in the US.To be more specific, the total medical expenditure has accounted for as largeas 17.7% of its GDP. The figures have been higher than 10% in all Europeancountries and above 8% among BRICK. However, it is only 5.89% in Sichuanprovince, which also demonstrates our huge potential.
2) Advance thesocial progress. People’s physical state represents the social and civilizationlevel of a country. The development in health service will not only improvepeople’s health, ease individual, household, and social burden caused bymedical bills, but also help us face the accelerate aging speed, and promotethe sound development and progress in both economic and social sectors. It issuggested by a study that the 8-l0% world economic growth in the past fourdecades is attributed to the healthy group of people whereas the 30-40%unprecedented progress of Asian economy is owing to the improved physicalhealth of the Asian population.
3) Acceleratethe transferring of development models. As one of a sectors in tertiaryindustry, according to the international and domestic experience, healthservice strongly drives the growth of other sectors, such as e-commerce,medical insurance, healthy tourism, catering, retailing, etc in the tertiaryindustry; raise the proportion the tertiary industry of GDP; and also encouragethe development of both first industry like plantation and secondary industrylike bio-pharmaceutical, facility manufacturing, and construction. Currently,the tertiary industry accounts for 60% of GDP at an average level while thefigure is only 35% in our province. As a result, the development of healthindustry presents an effective way to transfer the economic model featuringfrom investment-driven to consumption-driven.
4) Achieve sustainabledevelopment. As a fundamental and therefore rigid demand for human beings,health service, with continuous upgrading, market expanding, and expenditurerising, will become a pillar industry for economic development in future. Underthe background of global financial crisis, the industry, as the new normal, hasimmune from the reduced growth rate in the first and secondary industries,which fully demonstrates its sound anti-fatigue potential and industrialdriving effect. Meanwhile, health service has played an irreplaceable rolesince it accompanies us till the very last moment of life.
2.Currentconditions:
1)Massivedemand for health service
First,people’s awareness towards health has been greatly enhanced. With increasinglyimproved living standard, people care more about their health conditions, takeinitiative to seek for health advisory, adopt healthy lifestyles, and purchasehealth-care products and service. All of these have stimulated the development ofseries relevant industries.
Second,the aging and changed diseases have multiplied the demand for health service.In 2000, our province has stepped into the aging stage in advance. Geriatricdiseases and chronic but non-communicable diseases, in need of life-long andtaking-up health service, have become the major threats that pose challengesfor the health of our citizens. With the constant advancing of medicaltechnologies and ongoing improvement of the treatment methods, the chance forpatients with severe diseases to survive in the long term has been raisednotably; and the life expectancy has been expanded continuously. Thus, thedemand for health service has been on constant expansion as well.
Third,the growth of resident income will encourage the diversified services. As themoderate-income group is constantly increasing, and consulates as well asforeign enterprises are setting in Sichuan gradually, the demand for premium,high-tech and tailored health service is expanding; the service is catching upwith the tide of world development. And these have driven the development aswell as reform of the health service in our province.
2)Medical service is developing rapidly.
First,in our province, the number of (assistant) practitioners and registered nursesin medical institutions was over 170,000 and 150,000 respectively by the end of2013, with an increase of 13% and 30% compared with the figures in 2010. Amongprivate hospitals, the growth rate of (assistant) practitioners and registerednurses reached as high as 48% and 68%, accounting for 21% and 17% respectivelyof the total.
Andsecondly, in our province, the number of beds was over 420,000 in medicalinstitutions by the end of 2013, an increase of 41% compared with the figure in2010. Among private hospitals, the growth rate of increased beds was as high as85%, accounting for 23% of the total in our province.
Third,430 million visits to hospitals and 10.62 million discharged patients wererecorded in 2013, with an increase of 21% and 36% respectively compared withthe figures in 2010. In private hospitals, the visits to hospitals and thenumber of discharged patients grew by 90% and 153% respectively, accounting for15% and 20% of the total services.
3)The industrial layout has taken shape at preliminary stage.
First,the preliminary medical service network has basically covered the urban andrural areas. By the end of 2013, the numbers of medical institutions, beds,practitioners and registered nurses in our province have been 80039, 420thousand, above 170,000, and 150,000, respectively. The number of beds and(assistant) practitioners for per thousand people reached 5.51 and 2.22respectively, higher than the national average. The number of registered nursesfor per thousand was 2.02, higher than the average in Western area. The healthservice network has covered all the urban and rural areas, providing theguarantee for meeting the basic demand for health service.
Second,the leading enterprise in the industry has led rapid development of high-techand high-end service. Boasting a number of national and provincial medicalcenters including West China Hospital of Sichuan University, West ChinaHospital of Stomatology, Sichuan Provincial and People’s Hospital, our provincerank the first place in term of the premium medical resources in western area,leading the development of high quality resources and high-end service.
Third,private capital is encouraged to enter into the market rapidly to promote thediversified service. Under the incentive of medical reform, a series of well-knownbrands based on private capital has been fostered. Besides, domestic andoverseas capital in large amount has entered into the market of Chengdu.Providing medical service in diversified forms has achieved remarkable success.The innovated service models in line with the demand for modern serviceindustry are emerging. With the deepening of medical reform and the advance ofurbanization, our province has innovated a range of new service models andservice framework, which can be seen from the following aspects: the specialistclusters with representatives of Tianfu New Area, Chengdu International MedicalTown in Wenjiang district, and Health Town in Luzhou are taking shape andpromoting intensive development of the medical service; the collectivize andinternetize hospitals based on the large-scale hospitals in urban area are evolvingrapidly, advancing the process of “all-in-one-chain” service; the healthmanagement service model based on people’s health is taking shape, stimulatingthe taking-up service model integrating medical treatment and prevention.
4)The supporting conditions have taken shape.
First,the supporting conditions to the medical and pharmaceutical industries have enjoyedsophisticated development. The medical, pharmaceutical and medical apparatusindustries have shown the trend of cluster and part of enterprises has gainedthe competitive edge in the industry. For example, SICHUAN KELUN PHARMACEUTICALCO., LTD has become the largest infusion manufacturer and operator among theglobe; and Alltech has become the fourth among the world and the first in Asiato design, manufacture and distribute the Magnetic Resonance Imaging (MRI)systems.
Second,the information technology in the region has taken a lead in Western areas.Currently, with several top500 IT companies including Microsoft, IBM, andIntel, setting their branches in Chengdu, the IT industry has been identifiedas one of the high-end growing industries, receiving the cultivation priorityfrom the province. Additionally, public hospitals above the county level haveachieved information and network management in our province. A regionalplatform for collecting population health information is under construction.All of these have created favorable conditions for the integration of healthservice and information industry, and for the cultivation of new businesslayout.
Third,the medical education and training system has been complete. With 70,000students recruited each year, our province boasts 9 medical colleges anduniversities of higher learning like West China Center of Medical Sciences,Sichuan University, 8 vocational colleges, and 22 secondary vocational schools.This has provided sufficient, professional, and medium as well as high endmanpower reserve for the entire industry chain.
3.Development thinking, principle and target
1)Development thinking
Upholdingthe principle of health-oriented demand, taking reform deepening as drivingforce, and promoting medical service as guidance, we should give full play ofthe medical service in health; elderly support and insurance services;encourage private capital to invest in health insurance industry; activelyadvance the elderly support industry via the integration of medical treatmentand care; strongly cultivate the New business layout, technologies andproducts; make endeavor to expand supply; promote healthy consumption; furtherconsolidate the status of Sichuan province as the center of health serviceindustry in the West; and give an impetus to the socioeconomic transition.
2).Development principle.
Thehealthy service industry should stick to people-oriented policy and promotecoordinated development. It should firmly follow the guideline of providingquality health services, earnestly safeguarding the people's vital interestsand distinguishing basic health services from non-basic services to achievecoordinated development between the two. The industry should be driven bymarket with government guidance. Government should allocate urban, rural andregional health care resource to achieve promotes balanced development.Government should strengthen its role in institution building, planning, policydevelopment and regulation, give full play to the market in allocatingresources, stimulate social vitality, increase the supply of health services,and improve service quality and efficiency. Government should deepen reform,innovation and development, strengthen scientific and technological support,expand service scope, encourage the development of new formats, improve healthservice standardization and specialization, and set up healthy service sectorinstitutional mechanisms in line with the actual situation of the province,demands of sustainable development.
3) Goal of development
First, we will make a great development in health care service. By makinghealth management the main service mode, combining precaution, treatment andrecovery of diseases, we are to build a health care service chain. With thehelp of the development of health industry, the total hygiene cost is to reach200 billion yuan in 2017 and 300 billion yuan in 2020 and the new employment inhealth care service is to reach 400 thousand.
Second,we will increase the proportion of service industry value in GDP. Theproportion of health care service value in GDP is to be above 7% in 2017 andabove 9% in 2020. Its contribution as the third industry to the proportion inGDP will achieve an all-round increase.
Third,we will lead private funds into greater investments. There will be 5.7 beds perthousand patients in hospital and health care institutions in 2017; the amountof medical service provided by private-owned hospitals will account for 22% inthe total amount of the whole province.
Fourth, we will facilitate the constant development of high-tech sector. Basedupon the spread and application of a large number of advanced technologies suchas genetic diagnosis, organ/tissue transplantation, artificial breeding,polymer synthesis, laser technology, interventional treatment, etc., themedication research and development sector will be enhanced in order tostimulate the development of related industries and form a virtuous interactivecircle, therefore provides a long-term support for the demand of medicationsector.
Fifth, we will increase the utilization of medical service. Since the medicalinsurance system has covered all population, the health care service networkgradually improves, self-service capacity of grass-roots enhances constantlyand people (especially aged people) s potential demand for health careincreases, we will achieve that in 2017, there will be about 9 clinictreatments per capita in Sichuan province and 18 in 2020.
Sixth, we will promote people’s health level. We will achieve that in 2017,health care services for aged people will cover all aged population at home andabove 30% comprehensive hospitals above second grade will set up aged diseases sector.The average life expectancy will reach 77.2 years old in 2020, which surpassesnational average. The death rate of infants and pregnant woman/puerperal willdecrease to below 10 and 25/100000 respectively,which reaches national middle level.
Seventh, we will improve the circumstance for the development of servicesector. In 2017, the policy support for health care service sector will beperfected and rules and laws for this sector will be greatly improved. In 2020,government’s supervision and this sectors self-discipline mechanism will bemore effective; people’s conscience and attainment on health care will beremarkably improved and a better environment for the participation of the wholesociety and the development of health care service sector will be formed.
4. Key tasks
1) Enhance basic medical services
1. We will strengthen basic medical service system. We will change servingstyle, improve medical service qualities, adapt development structure andrealize a good match of medical resource and its capacity. By scientificmanagement, we will ensure that each county manage well 1 or 2 county-levelstate-owned hospitals, each city manage well 2 or 3 city-level state-ownedhospitals and several province-level large hospitals, hence ensure peoplesbasic medical demand. We will regulate reasonably the rate of comprehensivehospitals and specialized hospitals and strive to develop hospitals forchildren, mental patients and terminal care centers.
2.We will develop diversified high-quality medical resources. We will developregional medical service with the support of ministry-level or provincial-levelbackbone hospitals like Huaxi hospital of Sichuan University, SichuanProvincial peoples hospital, Huaxi No.2 hospital, Huaxi Oral Medical hospital,Sichuan Tumor Hospital, Affiliated hospital of Chengdu University of TCM,Sichuan Orthopedics hospital, etc. Encourage chain business and medicalinstitution group and enlarge provision of high-quality medical services.
3.We will facilitate social capital to provide non-profit services. We will makea rational arrangement of the quantity and scale of state-owned hospitals tomake enough room for private-found hospitals. We will lead social capital to non-profitmedical institutions and encourage private funds involvement in the reform ofstate-owned hospitals. We will enact practical policies to solve problems inthe development of private-fund hospitals like difficulties in implementation,financing, operation and development. We will release the examination ofsecond-class large equipment; improve rules for doctors to practice in variousplaces as well as the mechanisms for state-owned hospitals to supportprivate-found hospitals.
4.We will spread Chinese medical care services. Take full advantage of Chinesemedicine and promote with great effort Chinese medical care services and carryout actively prevention of diseases and researches on aged diseases. Encouragemedical institutions to provide Chinese medical services like identification oftemperament, conditioning sub-health situation, seasonal health care andprevention of diseases, etc. Set up Chinese medicine innovation mechanism anddevelop a group of Chinese medical health care product.
2) Enhance special medical services.
1. We will support the development of assembled medical services. With TianfuNew Zone in Chengdu as the platform and by developing provincial-level advancedmedical resources and introducing hospitals and hospital management companiesreaching international level home and abroad, Tianfu New Zone is to construct amodern, high-end medical service center which is first-class in western areaeven in the whole country and radiates the whole country and some foreigncountries. We will build Wenjiang International Medical Research Metropolis andLuzhou Health Care Metropolis as health care service center in Sichuan provincewhich radiates surrounding provinces.
2.We will support private-found special medical services. We will encourage andlead non-state-owned medical institutions to offer special medical care such ashealth checkups, nursing and recovery, terminal care, etc. Through overallplanning of the medical resources, we will build a multi-layer medical servicesystem adaptive to social need.
3.We will support medical cosmetic services; nurture and develop medical cosmeticservice market; provide diversified cosmetic services such as medical cosmeticservices, daily beauty services, image design, healthy diet program and so on;develop an innovated cosmetic economy based on technological, brand, qualityand service innovation; develop with great effort non-surgical cosmeticprograms based on cosmetic surgeries; promote a new management style combiningmedical cosmetic surgery and daily beauty services; consolidate and upgrade toa new level Chengdu’s statue and function as the first cosmetic city in Chinaand promote the coordinated development of beauty industry, entertainment andtourism industry.
4.We will support diversified health care services. Taking contemporary medicalservice sector and sector of medical equipment and biological medicine as a core,we will construct an industrial base concerning high-end medical servicecluster, high-tech medical equipment and biological medicine. We will developwholeheartedly a specialized center of medical examination, inspection,medicine researches and clinical trials, a medical service evaluation systemand a professional incubator for medicine industry for the purpose of buildinga public service platform for medicine technology innovation andintensification of medical treatment resources.
3) Improve elderly health care service.
We will make a reasonable arrangement of aged diseaseshospital, elderly nursing care center, elderly recovery and healing institution,terminal care hospital, etc. We will encourage transformation of hospital oradding elderly care institution. We also encourage comprehensive hospitals toprovide aged diseases sector, adding beds for aged patients and terminal carepatients and promote terminal care services. We will develop health careservices for aged people in communities and encourage hospitals and communityhealth care centers to extend their services to residents household. Thecommunities will set up records of aged people’s health conditions and developday by day a service contract with aged people’s family in order to offer dailycare, full-time nursery, part-time nursery, etc. We will drive the combinationof medical treatment and nursing care, promoting medical resources to get intoelderly nursing centers, communities and families. We will build and improvethe cooperation mechanism of medical institutions and nursing institutions,encourage advantaged nursing centers to set up medical institutions, offer agreen channel for the appointment with doctors in nursing centers and hospitalsand start a trial of long-distance medical services facing the nursing centers.
4) Promoting physical examination and consulting services
We will actively promote physical examination, and develop theservice industry around the platform of physical examination center. We willprovide services such as physical examination, consultation and maternal andchild care relying on health management organizations, and introduce relevantmanagement measures. We will vigorously carry out health counseling services.For the changes in disease spectrum, we will rely on the existing mental healthorganizations to provide health counseling and health-careservices such as psychological intervention, psychological comfort, andpsychological counseling. We will vigorously promote the "OverallIndividual Service" model, using a variety of diagnostic modes tocomprehensively analyze every system disorder, effectively implementingindividual health education and health promotion technology, more fullyunderstanding, analyzing and caring about the physical and mental conditions,and social conditions of our patients.
5) Improving health insurance services
To improve the basic medical insurance system and steadily enhancethe level of basic medical insurance, we will encourage commercial insurancecompanies to provide diversified, multilevel, and standardized products andservices. We will steadily promote serious illness insurance for urban andrural residents, continue to promote the medical liability insurance, anddevelop medical operation insurance such as accident insurance. Hopefully, by2017, the medical liability insurance rate of our province's secondary andhigher level medical institutions could have reached 100%. We will entrustqualified commercial insurance agencies to carry out various types of healthinsurance counseling and handling services via government procurement ofservices. We will strengthen the supervision of medical practices and controlof medical costs, and establish a cooperation mechanism between commercialinsurance companies and agencies of health-care, physical examination, andnursing services.
6) Carrying out online medical services
We will scientifically plan and prioritize the development ofonline medical service as a new strategic focus. We will build our province'shealth management information platform, accelerate the popularization ofresidents' health card, and promote the development and applications of distalend monitoring medical devices, wearable smart devices, and mobile medicalinformation system. On this basis, West China Hospital of Sichuan Universityand Sichuan Provincial People's Hospital long-distance medical treatment systemis set as our pioneer, hopefully becoming the first organization in China torealize data exchange and sharing between relevant services, agencies andindividuals as soon as possible. We will promote service model innovations inareas such as service appointments, individual health-care, professionalconsultation, and long-distance medical treatment. We will develop new healthindustry formats such as "Smart medical service" and"Personalized health service", and promote relevant informationindustry chain development such as financial IC card, domestic chip, cardmanufacturing, and IT equipment, so as to take the leading role of onlinemedical services and mobile medical development in China’s central and westernregions. We will promote more health monitoring system into families. Byrelying on primary level health care institutions, we will build a healthmanagement platform based on residents' health card, the Internet and cloudcomputing to provide long-distance medical treatment and health managementservices such as prevention, care, monitoring and calling.
7) Deepening public health services
We will promote the construction of disease prevention and controlorganizations, and speed up infrastructure construction and equipmentconfiguration. Hopefully, by 2017, all Center for Disease Control (CDC) of ourprovince's cities (prefectures) all have met standards above middlefirst-class; by 2020, CDC in counties whose population are millions all havereached the middle first-class standard, CDC in counties whose population aremore than 500,000 all have reached the upper second-class standard. We willaccelerate the construction of professional mental health organizations.Hopefully, by 2017, we could have jointly built the "Mental Health Centerof Sichuan Province" by the province and municipal cities. We will beinclined to support the construction and development of regional mental healthcenters, encourage social capital to be invested in mental health hospitals,and improve the prevention and control system. We will strengthen the functionsof vaccine for infectious disease prevention. We will pay close attention tothe work of the national expanded program on immunization, steadily improvevaccination rates, and build a strong immune barrier. We will strengthen thepreventive vaccination of II vaccines: expanding its vaccination range,increasing vaccination rates, and actively encouraging eligible regions toinclude vaccine against rabies, influenza, pneumonia in the scope of medicalclaims, and improving the development of related industries such as vaccine andplasma products production.
8) Promoting health education services
We will include health education into the economic and socialdevelopment plan. By playing a leading role, the government will create asupportive policy environment, increase capital investment, and establish a multi-sectorialcooperation and coordination mechanism to mobilize the community to participatein health education. Relying on agencies, schools, communities, socialorganizations, enterprises and institutions, we will vigorously carry outhealth education, and actively develop professional websites, professional TVprograms, professional broadcasting columns, video, books, audio and videoproducts, animation products and so on, promoting the development of healtheducation-related industries. We will improve the development of healtheducation institutions, their personnel, funding and technology. Hopefully, by2020, we could have established a relatively complete health education servicesystem adapted to social development.
9) Promoting reproductive health services
We will support medical and health institutionsand birth control technical service institutions to offer family planning,prenatal and postnatal care consultation clinic and carry out prenatal andpostnatal care counseling. We will comprehensively promote free pre-maritalmedical examination and pre-pregnancy health checks, early diagnosis andstandard treatment of hereditary diseases for urban and rural residents. Wewill improve the free service of having another baby for only child death anddisabled couples, and strengthen human assisted reproduction services. We willcarry out a health care project for emigrants, guiding them to have a physicalexamination before emigration. Relying on primary level health-care and familyplanning technical service institutions, we will focus on building 6-8 earlychildhood development demonstration bases before 2020, and on guiding citizensto actively participate in and accept the 0-3-year-old early childhoodeducation services. Before 2020, we will focus on cultivating 1-2 populationhealth management consulting firms.
10) Improving services of related industries
By developing health service industry, firstly,we can promote the integrated development of physical exercises, health-careand culture, increase the supply of sports products and services, developsports service industries such as tourism and sports rehabilitation, promotethe health and fitness training equipment industry, and cultivate more forms ofphysical fitness organizations. Secondly, we can drive the adjustment of healtheducation structure, promote the development of our province's health-carevocational training institutions to improve health-related educationindustries. Thirdly, we can promote the industrial integration of health andtreatment of traditional Chinese medicine, herbs growing, tourism, and fitnessin our province. Fourthly, we can promote the development of diversified,multi-level and standardized medical and health insurance products and servicesinnovations. We will support and develop a variety of medical device industriesclosely related to families, and let them gradually become advantageousemerging industries.
5. Forwardmeasures and responsibility divisions
1) Improving the health service development plan.Compile the "Thirteenth Five Year Plan" for Sichuan Province’sHealth Industry. Firstly, Clarifying the health service resourcesallocation criteria, principles and layout of our province, and comprehensivelyplanning various public service facilities; Secondly, fostering and encouragingthe expansion of high-quality medical resources, making them more extensive andstronger; Thirdly, encouraging the rapid development of private and foreignhospitals, and supporting the development of high-end medical services;Fourthly, promoting industrial integration of medical services, healthmanagement and information technology and fostering new formats; fifthly,actively developing professional medical care, health management,rehabilitation and retirement centers, traditional Chinese medicine and otherservices, encouraging the development of health care nursing homes, andextending the health service industry chain.
2) Increasing efforts to open markets. We willfurther expand market access, conducting "unlimited entry". Whereverthe law and regulations do not explicitly banned should be open to the publiccapital and we should increasingly expand the open areas. We will loosenrestrictions on the number and scale of non-public medical institutions, aswell as the configuration of large-scale medical equipment. Information oninstitutional settings and layout adjustment will be timely released. We willstrictly control the approval time of the establishment of medicalinstitutions, and encourage qualified regions to determine the handling body bymethods like bidding.
